The My Appointment Card is designed for you when seeing a new health professional or if you want to communicate or keep track of important information before and after an appointment.

The How to Leave an Appointment card has practical tips and strategies to recognise when you feel unsafe, disrespected or ignored in a medical appointment. It contains straightforward steps to maintain your boundaries and safely end and exit an appointment on your terms.
